We collect most of your information over the phone or in person. You will be asked to sign a demographic sheet and HIPAA form.

We accept most major insurance providers in the region. If you’d like to verify your coverage before your appointment, just call the phone number on the back of your insurance card or call our office Billing department at 971-772-3003.

Your co-pay amount is determined by your insurance plan. In many cases, it will be listed on your insurance card. We recommend contacting your insurance provider before your visit to confirm the amount. Please remember that co-pays are due at the time of your appointment. We are also happy to set up payment plans for those who have high insurance deductibles. Our billing department will be happy to set that up with you!
