New Patient Information
We are so glad you have chosen the staff of Woodburn Pediatric Clinic to partner with you and your family as we work together through your child’s growth and development.
Preparing for your visit
Many children become nervous or anxious when it’s time to visit the doctor. They are afraid that their parents might leave them alone with the doctor, that the doctor will hurt them, or simply of the unknown. Encourage your child to express their fears. Discuss the purpose of the visit and tell your child what to expect. This will help make the visit a positive experience.
Always schedule a visit as soon as you know your child needs to see a doctor. Your child needs regular check-ups and physical examinations, so always schedule these appointments well in advance. If your child is sick, same day appointments are always available. We are better able to accommodate a same day appointment if you call as soon as you can in the morning.
For your first visit, plan on arriving about 15 minutes early to complete a health history form.
The Woodburn Pediatric Clinic will directly bill your claims to most insurance companies. All balances are due within thirty days from the date on your statement.
If you need to make other arrangements, please contact our Business Office at 503.981.1280. We can potentially offer you monthly payment plans to help you pay your balance due.
You will need to bill your insurance directly for an visit related to a motor vehicle accident, traveler’s insurance, foreign country insurance, or a third party liability claim. We will provide you with a CMS-1500 form and receipt upon request. Your third party insurer will need this information to process your reimbursement. For these types of visits, payment is due in full at time of service.
We will accept patients without insurance. We accept two of the OHP plans: MPCHP (Marion Polk Community Health Plan) and CareOregon. We also accept Family Care Health Insurance and offer a sliding scale discount for patients with no insurance.
